* Special Discount for Instructors : $10 Discount per each Student registered and paid full fee. (Instructors print Registration Form fill names of students to qualify, and make sure each student fill their own registration form and pay fee).

Dance is responsibility to and care of Self!.

In consideration of services received as a student on these premises, the undersigned hereby releases and forever discharges Alborea Entertainment, LLC., Alborea Dances, their instructors, guest instructors, hosts, studio, their heirs, successors, administrators, and assigns from any and all actions, cause of actions, liability, claims and demands upon or by reason of any damage, loss, injury or suffering known or unknown which may be sustained in connections with and in course of training through Alborea Entertainment or host company or any instructor or Guest instructor or fellow student acting on behalf of Alborea Entertainment or for the course of training.

The undersigned assumes all risks inherent to this type of physical activity, any loss(es), or damage at host studio or in parking lot to self or belongings and will respect all in the Alborea Entertainment property and/or Studio, including other’s property, including copyright materials of Alborea Entertainment or associates and trade secrets/specialties/concepts of Alborea Entertainment. If you are dancing in Alborea Entertainment/Alborea Dances productions, you agree not to be in competition with Alborea Entertainment/Alborea Dances or to use Alborea Entertainment/Alborea Dances specialties for other performances non-related to Alborea Entertainment/Alborea Dances without permission.

The undersigned also states that he/she is in good physical condition and assumes all risk and has permission from a medical health professional to participate in such level of physical activities and classes.

The undersigned further agrees that his/her likeness may be used in future promotional/commercial materials.
